All,
I remember talking about this question at the time of DCRM(B)
publication, but can't locate the rule.
In a situation where Chez appears once but is clearly meant to apply
to both names following a left curly bracket, would we supply [Chez]
in transcribing the second name?
Randy Brandt has supplied a link to an example (THANKS, Randy):

(Attn: Nina and Eduardo - This would be a good discussion point for
the Examples to Accompany DCRM(B). I have a book with six names
following a left curly bracket!)
Thanks,
Annie Copeland
