Here are additional comments on the Introduction not previously mentioned:

1. Page 15, Intro 5. Language preferences, last sentence: "See ... and
areas 5 and 7."
I could not find the word "English" anywhere in Area 5. We could remove
reference to that area in the phrase to read: "See ... and area 7."

2. Page 17, "Local note"
Change "(see 2B11)" to "(see 2B11.2)"
