In a way it resembles modern "lift-the-flap" books, which are usually for children and have flap with an image (or text) concealing another image (or more text).
But for description I wholeheartedly agree with Richard!

Start with the dimensions of the leaf before folding and pasting; it's folded into x sections, which can be referred to as sections 1-x; then describe which ones are pasted together, and what's on them all. In a catalogue record of the basic physical structure (MARC 300) I'd call it "1 folded sheet", and present the detailed description as a note to the collation.
The binder must have been so grateful for the challenge.

Dear Deborah,
I have another cataloguing question for you: how would you describe the attached (on a Dutch almanac from 1799)? It’s not quite accordion/concertina fold. The leaf has been folded into eight sections, some of which have been pasted back-to-back, the others pasted down to the inside cover of the book. I hope the photos illustrate what’s going on. Let me know if you’d like more.
