[DCRM-L] Standard Citation Forms May 2019 update

The Standard Citation Forms Editorial Team has made some changes to the "Submit a Proposal" form on the SCF site (http://rbms.info/scf/). The purpose of these changes is to streamline our work on the back end of the database, but due to some limitations with WordPress the form you see when you submit a new citation form has a lot more fields than the old one. The same number of fields are required for submission as before; the rest are all optional.
