[DCRM-L] approved DCRM change proposals

My understanding from the BSC discussion was that these changes were
primarily for the benefit of forthcoming modules, in order to discuss
more widely and document the kinds of incremental changes that were
being made anyway (for example, the wording changes that DCRM(S) made to
the "505" section of Appendix B, and to Appendix D3.2). 

Also, keep in mind that the replacement pages created for the 2nd
printing of DCRM(B) do not reflect all the desired changes, only those
that could be made if the edit did not affect page breaks.
