[DCRM-L] looking for term for printing error not listed in RBMS printing & publishing terms

Hello,

I am cataloging an edition Actiones et monimenta martyrum (Crespin, 1560,
4to) and ran into a curious printing error in our copy that I am having
difficulty finding an appropriate RBMS printing term for. The majority of
the gatherings are in 8s, except R, which should be an 8, but in my copy is
in 12.  Signatures R3 and R4 take up 8 leaves total, each printed on one
side only. My best guess is that this resulted from two sheets of paper
being stuck together and run through the press. I have constructed a local
note explaining the error, but would also like to add a 655 from a
controlled vocabulary. Has anyone else encountered this type of error? If
so, how did you a) describe it, or b) were you able to find a term from the
RBMS printing terms or other controlled vocabulary to add?
