I agree. Treat it as a quoted title and capitalize the first letter.
I see DCRM(B) avoided the issue by having the a chief title that starts with a proper noun: Hereafter foloweth a litel boke called Colyn Cloute
And DCRM(G) does the same end-run, with a poster title from a very different era: Bill Graham presents, in San Francisco, Grateful Dead, Junior Wells Chicago Blues Band, and the Doors

Isn't this one of those questions that regularly comes up? I thought it was a capital The because it's not a possessive and so more like a quoted title. In any case, that's what I would do.

OR
Here beginneth a good boke of medecines called The treasure of pore men
That is, is the chief title, which is embedded in the title proper, more like a quoted title, or more like Eileen Ford's a more beautiful you in 21 days?
