Is it possible you saw
http://gateway.proquest.com/openurl?ctx_ver=Z39.88-2003&res_id=xri:eebo&rft_id=xri:eebo:citation:99857102
, which has the note "Identified as STC 23084 in UMI Microfilm"?

FWIW, Hamnet says the Folger has four copies (
http://hamnet.folger.edu/cgi-bin/Pwebrecon.cgi?BBID=169689) so if there's
something in particular you'd like me to check and snap a photo of with my
phone, let me know.

Erin.
